Goal-orientated, comprehensive, co-ordinated and multidisciplinary care program for an patient immediately after or instead of acute hospitalisation for an acute illness, injury or exacerbation of a disease process requiring frequent patient assessment of the clinical course and treatment plan, and the duration of which is a limited period of time determined by the time taken for a condition to stabilise or fo completion of a predetermined course.
