Sub-Acute Care
Goal-orientated, comprehensive, co-ordinated and multidisciplinary care program for an patient immediately after or instead of acute hospitalisation for an acute illness, injury or exacerbation of a disease process requiring frequent patient assessment of the clinical course and treatment plan, and the duration of which is a limited period of time determined by the time taken for a condition to stabilise or fo completion of a predetermined course.
Advanced Wound Care
Our advanced wound care program is designed for patients who are discharged from acute hospitals with wounds or ulcers that have not completely healed or with new ostomies.
Intravenous Infusion Support
Our infusion support program is the administration of medications or nutrients directly into the body through a blood vessel, under the skin, or into a muscle.
Oncology Management
Our oncology management program is a doctor-supervised program for patients who have undergone treatment for cancer and designed to help patients return to activities of daily living.
Pulmonary Management
Our pulmonary management program is a doctor-supervised program for people who have chronic lung diseases. The program is also often used before and after lung surgery.
Surgical Recovery
Our surgical recovery program is a supervised outcome based program focused on improving a patient's recovery after surgery.
